چکیده
SUMMARY Thirteen patients with left ventricular aneurysm due to coronary heart disease were studied by left heart and coronary sinus catheterization, including cineventriculog-raphy and measurement of ventricular mechanics and energetics at rest, and in some subjects, during either isoproterenol infusion or supine leg exercise. Eight patients had an aneurysm estimated to comprise greater than 20% of the left ventricular surface area, associated with increased left ventricular end-diastolic volume and pressure and mean systolic force. Average isometric rate of pressure rise and mean fiber shortening velocity and distance were uniformly decreased. Five patients had an aneurysm, estimated to comprise less than 15% of the left ventricular surface, associated with normal or nearly-normal left ventricular end-diastolic volume and pressure and mean systolic force. Average isometric rate of pressure rise was normal, but fiber shortening velocity and distance were moderately depressed. Stroke output and cardiac output were reduced in both groups. Aneurysms exhibited either paradoxical systolic expansion or apparent lack of motion (akinesis), or both. A theoretical analysis presented indicated that when approximately 20 to 25% of left ventricular area is inactivated by any pathological process , the degree of shortening distance required of the myofiber to maintain stroke volume exceeds physiological limits, and cardiac enlargement (Starling mechanism) must ensue to maintain adequate ejection of blood. The magnitude of the salutary response of isoproterenol coupled with an increase in mechanical efficiency during catecholamine infusion suggested that myocardial cate-cholamines were depleted with additional aggravation of heart failure in this disease.

Right ventricular Hemodynamic Alteration after Pulmonary Valve Replacement in Children with Congenital Heart Disease
Introduction: In patients who underwent surgery to repair Tetralogy of Fallot, right ventricular dilation from pulmonary regurgitation may be result in right ventricular failure, arrhythmias and cardiac arrest. Hence, pulmonary valve replacement may be necessary to reduce right ventricular volume overload. The aim of present study was to assess the effects of pulmonary valve replacement on rig...
متن کاملSimulation of left ventricular function during dyskinetic or akinetic aneurysm.
The purpose of our study was to simulate the hemodynamics of left ventricular function after left ventricular aneurysm (LVA) of various sizes and to validate the results of this computer based simulation with patient data. We developed an equivalent electronic circuit (EEC) that reflects the hemodynamic conditions of LVA (after acute myocardial infraction) while taking into consideration the re...
متن کاملAneurysm of the membranous ventricular septum in transposition of the great arteries.
In patients with transposition of the great arteries (TGA), both the D- and L- forms, an aneurysm of the membranous ventricular septum (AMS) produces subpulmonic stenosis due to the higher right ventricular pressure which forces the aneurysm to protrude into the left ventricular, i.e., subpulmonic, outflow tract. The clinical signs and symptoms, hemodynamic findings as well as surgical results ...
متن کاملMapping of a postinfarction left ventricular aneurysm–dependent macroreentrant ventricular tachycardia
Introduction Activation mapping of ventricular tachycardia (VT) is rarely accomplished owing to limited temporal and spatial resolution, unacceptably long mapping time, and hemodynamic instability. Entrainment mapping is a reasonable approach to identify targets for ablation in patients with tolerated postinfarction reentrant VTs; however, it often does not allow delineation of the entire VT ci...
متن کاملRadionuclide and Hemodynamic Assessment of Left Ventricular Functional Reserve in Patients with Left Ventricular Aneurysm and Congestive Cardiac Failure
The hemodynamic response to exercise stress was assessed in 17 patients with left ventricular (LV) aneurysm, demonstrated by contrast left ventriculography, and congestive cardiac failure (CCF) before and after sublingual isosorbide dinitrate (ISDN). Radionuclide ventriculography was performed at rest and during exercise in 14 patients. ISDN increased mean exercise LV stroke work index (LVSWI) ...
